Form 433A is used when your case is assigned to an IRS Revenue Officer.  The IRS will want more information on your personal finances compared to Form 433F.

Form 433A is for individuals and self-employed businesses.  For all other businesses use Form 433-B, C Corporations, S Corporations, and Partnerships.
